RISE briefs frame and provide key insights about priority topics relevant to Ontario Health Teams (and comparable initiatives in other jurisdictions) as they undertake efforts to address acceleration priorities, adopt a population-health management approach to improve quadruple-aim metrics for their attributed population, and put in place and strengthen the Ontario Health Team building blocks.
