St. Matthew’s House (SMH) is a non-profit agency serving vulnerable seniors within the Hamilton area. SMH staff members serve as case managers to address the unique and evolving needs of their clients. These needs include, but are not limited to: housing, food security, healthcare, legal, and transportation needs. SMH staff members have identified several shortcomings in the monthly spreadsheets they use to collect, summarize and assess data from their clients. This report aims to identify the key problems in the current data collection system, and develop a new data collection tool that better informs the SMH staff members of the clients’ needs and how staff hours are allocated.en_US
