dc.descriptionCommunity Paramedicine at Clinic (CP@clinic) is a chronic disease prevention, management, and health promotion program for older adults held in subsidized buildings by trained paramedics. The CP@clinic intervention had a significant effect on mean monthly ambulance calls and a greater improvement in Quality-Adjusted Life Years (QALYs) in intervention buildings compared to control buildings. / Paramédecine communautaire à la Clinique (PC@clinique) est un programme de prévention, de gestion et de promotion de la santé des maladies chronique visant les personnes âgées vivant dans des immeubles subventionnés. Le programme est mis en place par des paramédicaux qualifiés. L'intervention de la PC@clinique a eu un effet significatif sur la moyenne mensuelle des appels d'ambulance et une nette amélioration des années de vie pondérée par la qualité (QALY) dans les immeubles d'interventions comparés aux immeubles des contrôles.en_US
