Abstract: As the older adult demographic grows, the Hamilton Niagara Haldimand Brant Local Health Integration Network (LHIN 4) faces the challenge of how to best address the resulting health and financial pressures.The evidence brief will identify: 1) current problems and their causes; 2) three elements of a potentially comprehensive approach for addressing the problems; and 3) key implementation considerations.
